GPM HSE PTW Online Assessment Management System

Permit to Work (PTW) system is an essential part of safe system of work. It provides steps for authorizing persons to carry out work while warning them of possible hazards and spelling out precautions needed for work to be done safely.

PTW system is important in Gulf Petroleum Myanmar (GPM) operations to ensure all its activities with potential to harm people, pollute environment, damage asset and tarnish reputation will be managed properly and safely.

GPM HSE PTW Online Assessment Management System is a secured, dedicated web-based system for GPM. The system is to be a common and official way of assessment for GPM PTW system. Through this system, GPM HSE will be able to monitor the PTW assessment status of respective individuals going to worksites.

TAKE EXAM

Level 1: Awareness

1. Duration 1-day
2. Assessment will cover topics below:

  1. Golden Rules & General HSE & Human Failure
  2. PTW Module;
  3. JHA Module;
  4. Associated Procedures
  5. §Confined Space Entry Module;
    §Energy Isolation Module; and
    §Bypassing of Safety Critical Protective Devices Module.

Level 2: Certification – Contractor

1. Duration 3-day
2. Assessment will cover topics below:

  1. PTW Module;
  2. JHA Module;
  3. Associated Procedures
  4. §Confined Space Entry Module;
    §Energy Isolation Module;
    §Bypassing of Safety Critical Protective Devices Module; and
  5. Golden Rules
  6. General HSE & Human Failure
3. His/her appointment letter should state:
  1. Approved to be a GPM PTW Applicant/user;
  2. Accountable for all related activities stated in the PTW; and
  3. Responsible to ensure PTW is implemented as per GPM PTW requirements.

Level 3: Certification – GPM Staff

1. Duration 3-day
2. Assessment will cover topics below:

  1. PTW Module;
  2. JHA Module;
  3. Confined Space Entry Module;
  4. Energy Isolation Module;
  5. Bypassing of Safety Critical Protective Device Module; and
  6. Golden Rules
  7. General HSE & Human Failure
   3. His/her appointment letter should state:
  1. Approved to be a GPM PTW Approver;
  2. Accountable for all related activities stated in the PTW;
  3. Responsible to ensure PTW is implemented as per GPM PTW requirements;
  4. Required to carry out PTW HSE Assurance as required; and
  5. Required to carry out daily PTW Inspection.
